GUIDE: Free Dementia Caregiver Support
A Medicare-funded program with up to 80 hours per year of in-home respite care for the caregivers of our dementia patients.
What is GUIDE?
The Guiding an Improved Dementia Experience (GUIDE) model is a Centers for Medicare and Medicaid Services (CMS) program that aims to reduce strain on unpaid caregivers and enable people living with dementia to remain in their homes and communities.
The program is funded by Medicare for enrolled patients. Memory Treatment Centers partners with Craniometrix to deliver this caregiver support, and provides individuals and families with an interdisciplinary team for care navigation, education, support and respite care.
Respite care is available for individuals enrolled in the program that have moderate to high dementia care needs and receive support from an unpaid caregiver, such as a family member.

What support is offered through GUIDE?
Memory Treatment Centers will still provide all medical care for the patient, including any medication, tests or other care related to the patient's dementia.

- Ongoing monitoring and support
- Medication reconciliation and management
- 24/7 helpline access
- Care coordination and transitional care management
- Referrals and coordination for services and support
- Caregiver education and support
- Respite services

What does Craniometrix do?
Craniometrix provides support for caregivers and helps them access services to meet the patient's social needs, including:
- Up to 80 hours of annual respite care funded by Medicare (subject to assessment)
- 24/7 access to advice and support
- Regular assessment of patient social needs and caregiver needs
- Education and support groups for caregivers
- Support accessing community services
